<p>Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and be determined as requiring a high level of service needs. </p>
<p><strong><span style="text-decoration: underline;">ABOUT THE POSITION:</span> Registered Nurse</strong></p>
<p><strong>Duties & Responsibilities</strong></p>
<ul>
<li> ACTT: Conducts Medical Assessments, LOCUS, Co-occurring Screens and Functional Assessments.</li>
<li> Collaborates with individuals to create individualized, person centered treatment plans and documents clients progress during treatment.</li>
<li>Provides individual and group education around self-care, medical conditions, mental illness, and medications..</li>
<li>Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions</li>
<li> Develop crisis plans for clients and work with other staff to respond to crisis situations as needed</li>
<li>Provide medication administration for injectable medications and other medication monitoring and coordination needs.</li>
<li>Collaborating with Primary Care, Hospitals, Visiting Nurses and Specialty Medical Providers in the community.</li>
<li>Provide education, support and coaching around skills of coordinating self-care and medical/behavioral health care needs.</li>
<li> Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engage and communicate with family and friends,<br />be safe in the community, care for children and other family members, and solve everyday problems that arise.</li>
<li>Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources</li>
<li>May perform other assignments and duties, as area needs require.</li>

<p><strong>Education:</strong> Registered Nurse, Bachelors level preferred<br /><strong></strong></p>
<p><strong>Experience:</strong> 2+ years of experience with identified population<br /><strong></strong></p>
<p><strong>Licensure/Certification/Registration:</strong> Registered Nurse; Valid driver’s license required.</p>

<p>$63,634/minimum, and up commensurate upon experience</p>
<p>*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy.
